Lost or Stolen Debit Card

If your debit card is lost or stolen, please contact your branch of account immediately to secure a replacement debit card. 

If it is after business hours, please call 1-800-554-8969.

If you become aware of possible unauthorized or fraudulent transaction(s) on your account, please notify us immediately using the contact information referenced above. 

To expedite reporting of unauthorized transactions, please click on the link below and complete a Debit Card Chargeback Request Form for each unauthorized transaction. The form(s) must be completed in its entirety for each transaction and signed by an account holder.  You may mail or drop off the form(s) at any of our four locations.
